What are medication errors and why is it important to report them?

A medication error is any preventable event that occurs during the medication use process — from prescription to administration — and that may cause patient harm or lead to inappropriate use of the medicine, regardless of whether such harm actually occurs. These errors may occur while the medication is under the control of healthcare professionals, patients or consumers, and are characterised by being potentially preventable.
